1986: CHALLENGER DISASTER.  The Space Shuttle Challenger exploded 73 seconds after launch, killing Astronauts Francis R. Scobee, Navy Cmdr. Michael J. Smith, Dr. Judith Resnik, Dr. Ronald E. McNair, USAF Lt Col Ellison S. Onizuka, Gregory Jarvis of Hughes Aircraft Corp., and schoolteacher Christa McAuliffe.  This tragedy delayed America’s manned space program for more than two years.
The cause of the disaster was the failure of the primary and secondary O-ring seals in a joint in the right Space Shuttle Solid Rocket Booster (SRB). The record-low temperatures on the morning of the launch had stiffened the rubber O-rings, reducing their ability to seal the joints.
